Enrollment procedure:
In order to register please follow these 3 easy steps below:
1) Complete registration form and sign waiver
2) Provide deposits and post-dated cheques
3) Submit registration in person to office
4) No spots will be held until full payment is given with registration forms.
Payment schedule:
In order to secure a spot in a class at FCDA, the following must be complete:
1. A $100.00 non-refundable deposit per dance class, dated the day you register, accompanied by 4 post-dated payments, dated for the dates below:
September 1st 2011
October 1st 2011
November 1st 2011
December 1st 2011
2. Payments may also be made by one cheque in full at the time you submit your registration to the office.
3. Payment must be received in full for all students who register after December 1st, 2011.
Methods of Payments:
Refunds and withdrawals:
All deposits are non-refundable.
All costume fees are non-refundable after November 1, 2011
If a student withdraws, funds are not transferable
FCDA will only issue a monetary refund if withdrawing BEFORE OCTOBER 18, 2011.
A credit for the remainder of the year will be applied towards next year's classes if withdrawing between October 18th and December 1 2011.
If a student would like to withdraw from a class or the school, Front and Centre must receive a written or e-mailed confirmation dated letter in order to receive a refund or a credit. Notice of withdrawal must be provided strictly to Jodie or Sebastien. A refund will be issued only if withdrawing BEFORE 30 DAYS FROM 1st CLASS. A credit will be issued for all other matters for next years classes. ABSOLUTLY NO REFUNDS OR CREDITS AFTER DECEMBER 1ST, unless accompanied by a doctors hand written note, no exceptions.
